Registration Guidelines

All participants have to register via the Online Registration Form. Please read the instructions below carefully.

Each participant will receive a Registration number by e mail after the submission of the Online Registration Form. Please do not send any payment by bank transfer without the Registration number and participant’s name included in payment details.

Online registration will be closed on 28 August 2017.

Please note workshop registration is not included registration fee and has to purchased individualy.

All fees are stated including the VAT. All fees should be paid in CZK (Czech Crown), free of all bank charges.

* To qualify for the fee membership has to be valid until the end of October 2017.
**The official proof of student status has to be uploaded during the registration.

Please make sure to pay your registration fee before the deadline for the selected registration type. If your payment is not received before the deadline, the latter fee will be automatically charged. Unpaid or only partially paid registration will not be considered as valid until it is fully paid.

  • Half day guided tour – Grand tour of Prague

Social Events

Participants are asked to reconfirm attendance of all social events in the Online Registration Form. This also applies to the social events included in the registration fee. The booking of the various tours and events will be confirmed following payment on a “first come, first served” basis and numbers may be restricted.


Insurance

The registration fee does not include participant’s insurance against accidents, sickness, cancellation, theft, property damage or loss. Participants are advised to take out adequate personal insurance.


Confirmation

Each participant will receive the registration overview and email notification confirming the received payment and eventually stating the amount outstanding after the completion of the Online Registration Form.


Payment

All fees should be paid in CZK (Czech Crown), free of all bank charges.

By Bank Transfer

Please do not send any payments by bank transfer without the Registration number and participant’s name included in payment details.

The payment by bank transfer is available until 21 August 2017. After this date, all payments can be done only by credit/debit card.

Online Payment by Credit/Debit Card

In case of rejected online credit/debit card payment, the delegate is recommended to log into the Online Registration Form and try to process the registration and the payment again. Should the trouble with the payment persist, the participant is advised to contact the credit/debit card issuer/bank first and check if online transactions are not disabled on the credit/debit card. Afterwards, the registration can be processed again.

All online payments are processed via GP webpay secure connection. GP webpay conforms to international standards and it meets the strictest safety requirements of the MasterCard SecureCode and Verified by VISA standards as defined by the MasterCard and VISA card associations. Online Payment by Credit/Debit Card

Final Invoice

The final invoice will be provided to the participants only upon request, the Secretariat will not send this document automatically. Please contact us to ask for your receipt.

The final invoice will be issued based on details provided while registering (step Contact/Invoice details). Any change of such details (address, VAT number etc.) is possible only within 3 days from the registering date and is subject to the 500 CZK fee.


Cancellation / Refund Policy

Cancellation of Registration:
Until 20 June 2017 – 500 CZK administrative fee will be deducted from all refunds.
From 21 June 2017 until 10 August 2017 – 50% of the fee will be refunded.
From 11 August 2017 no refund can be processed.

Cancellation of Other Bookable Items:
Until 10 August 2017 – 500 CZK – administrative fee will be deducted from all refunds.
From 11 August 2017 no refund can be processed.

  • All cancellations must be notified in writing (by email or by fax) to C-IN and cannot be done online or over the phone.
  • Requests for refunds made after 31 October 2017 will not be considered.
  • For any change of name a fee of 700 CZK will be charged.
  • All registrations received by 28 August 2017 will be included in the list of participants.

Group Registration

Group registrations cannot be processed online under one email address identification; each participant needs to have his/her own profile set up with unique email address.
When registering more than one person under one company identity/invoice, please contact the Secretariat to determine the preferred invoicing setting.


Consent to the Processing of Personal Data

Officially Directive 95/46/EC on the protection of individuals with regard to the processing of personal data and on the free movement of such data

I agree to the collection, preservation and processing of personal data contained in this form, by the administrator, C-IN, Prague Congress Centre, 5. května 65, Prague 4, 140 21 (or its employees) for the purpose stated below.
This consent is granted for all of the information contained in this form, and throughout the five years from the date of consent. At the same time I am aware of their rights under § 12 and 21 of Act No. 101/2000 Coll. on the protection of personal data.
I am familiar with all completed parts of this form, and I declare that all information are accurate and truthful and are provided voluntarily.

Purpose of Processing Personal Data

In accordance with § 5 of Act No. 101/2000 Coll. on the protection of personal data, all information given on this form are collected and processed solely for purposes of processing registration and accommodation requirements for the ICUD 2017 and for sending related messages to the data subject via electronic means pursuant to Act No. 480/2004 Coll. Summarized data from this form may be used by the administrator for internal statistical purposes and for internal use of the administrator.

Statement by Administrator

Data administrator declares that they will collect personal data to the extent necessary to accomplish the intended purpose and may be processed only in accordance with the purpose for which they were collected. Employees of the administrator or other individuals who process personal data under contract with the administrator and other persons are obliged to maintain the confidentiality of personal data, even after termination of employment or work.


Responsibility

The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the event be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes. With registration, the participant accepts this proviso.
